HOLISTIC COMMUNITY THERAPY is seeking a Mental Health Occupational Therapist in the Portland metro area in Portland, OR. Flexible scheduling!

Note: This is a hybrid position and requires a mix of virtual and in person work in Portland, OR. 

We are seeking a mental health occupational therapist to perform adult (18+) assessments and facilitate development and rehabilitation of clients with psychosocial disabilities.

Occupational Therapist Job Responsibilities:

  • Provide in-home, community-based, and virtual services to 25 clients per week, 1 hour sessions within the Portland Metro area.
  • Perform functional and mental health assessments, including ADL, IADL, psychosocial, and cognitive assessments.
  • Evaluate client current functional status and home environment to develop individualized and measurable treatment goals.
  • Assess and interpret evaluations and test results to determine occupational therapy treatment plans in consultation with physicians or by prescription.
  • Plan and implement creative, comprehensive and clinically appropriate OT interventions using a client-centered, occupation-based approach to foster independence.
  • Identify, utilize and recommend appropriate community resources with the client's interests in mind.
  • Collaborate with the team to deliver comprehensive client care and contribute to practice-wide administrative and operational projects to support organizational goals.
  • Communicate frequently with staff and clients to align administrative needs with clinical workflows, fostering a cohesive and efficient practice environment.
  • Prepare clients for return to employment by consulting with employers; determining potential workplace accommodations; retraining supportive staff; and educating employers on workplace accommodations.
  • Complete discharge planning by consulting with other health care providers, including: physicians, nurses, social workers, counselors, and/or other relevant staff in organizations the client has been connected to.
  • Assure continuation of discharge plan by designing and instructing clients, families, and caregivers in OT recommendations, including recommending assistive equipment and/or recommending follow-up programs and services.
  • Document in the electronic charting system and effectively use Microsoft Office and other administrative tools to meet compliance and operational standards, ensuring accurate and timely reporting.
  • Protect clients by adhering to state and federal public health infection-control standards.
  • Adhere to mandatory abuse reporting laws, state and federal laws in service provision, including but not limited to: HIPAA requirements, QMHP requirements, and/or any additional requirements to maintain program funding.
  • Participate in the development and implementation of new program designs, revisions, and planning, taking initiative to identify and complete administrative tasks that enhance practice operations.
  • Engage in program evaluations and outcome studies for clinical interventions, leveraging data to support continuous improvement and operational efficiency.

Work Schedule : Full-time, 36 hrs/wk flexible schedule.

Compensation: $74,000/yr - 96,000/yr DOE.

Shift: Day, can include weekends (therapist defined)

Location: Work from home (WFH) position + commute to client location - 50/50 split

Education, Experience, and Licensing Requirements:

  • Current uncencumbered occupational therapist license in Oregon.
  • Certified Occupational Therapist (OT) by the American Occupational Therapy Association (AOTA) or Occupational Therapist Registered (OTR) by the National Board in Certified Occupational Therapy (NBCOT).
  • Competency or willingness and ability to gain competency in completing a DSM diagnosis.
  • Competency in providing both home/community-based and virtual services.
  • Minimum of one year of recent work experience as a mental health occupational therapist (preferably practicing QMHP competency standards).
  • Experience assessing and implementing clinical interventions related to mental health, other psychosocial issues and chronic disease.
  • Experience working with persons living with complex mental health and substance use challenges as well as working with underserved clients.
  • Experience providing home-based or community-based care preferred.
  • Current BLS/CPR for Healthcare Provider.
  • National Provider Identifier (NPI) and Taxonomy code required at time of hire.
  • Valid driver's license, access to a personal vehicle and auto insurance that meets minimum practice requirements for providing in-person services.
  • Strong organizational and time management skills, with the ability to balance client care and administrative tasks effectively.
  • Clear written and verbal communication skills.
  • Honor a diversity of beliefs and lifestyles and demonstrate culturally competent practices and an understanding of the effect cultural influences have on behavioral health services.
  • Proficiency in administrative tools or systems (e.g., scheduling software, documentation platforms) and a willingness to learn new systems as needed.
  • Demonstrated initiative and problem-solving skills, with experience managing projects or supporting business operations preferred.
